Cedar Mountain Commons - Basic Information

Independent or Assisted Living
Cedar Mountain Commons provides independent living apartments (no personal care or nursing services) and assisted living apartments (regularly scheduled/available personal care and nursing services) to its residents.
Cedar Mountain Commons does not have a secured neighborhood for candidates whose health and/or mental status requires restrictive living arrangements.
Resident Care Department Staffing
Certified Nurse Aide staffing is available on site 24 hours per day, 7 days per week. Nurse staffing is on site 40 hours per week. At all other times, a Registered Nurse is "on call".
AL in IL (Age in Place) Program
Residents of the independent living neighborhood who's care needs have changed can "age in place" and remain in their apartment. The program is referred to as "AL in IL". Residents interested in participating in this program must go through the same medical/nursing assessment process that residents of the assisted living neighborhood do. Once in the "AL in IL" program, residents receive 3 meals per day as well as scheduled/intermittent services (to include medication management) arranged by the Resident Care Director and coordinated through the Resident Care Department. Independent living fees are adjusted for participants in the "AL in IL" program.
Admission/Move-In Requirements
Cedar Mountain Commons requires a health and functional assessment by our Resident Care Director prior to making a decision about your move. This will help you to determine whether, or how best, your needs can be met. In order to be accepted for assisted living (personal care and nursing) services, your condition must be certified by your personal physician as being "chronic and stable". In addition to meeting the physical and functional requirements, we require certain financial information to determine your ability to pay for services and care.
Discharge/Move-Out Requirements
Connecticut law does not list specific conditions or circumstances that would require you to leave Cedar Mountain Commons. Decisions about move-out are made on an individual basis and always include you, your family (as appropriate), your personal physician or other legal representative. We will assist you with arrangements to move to another setting if necessary, however, making those arrangements is your responsibility, or the responsibility of your family or other representative if you are unable to do so. You are not required to move out of Cedar Mountain Commons if your condition is no longer chronic and stable, however, in those circumstances your health care must be provided by a licensed home health agency who will work with our Assisted Living Services Agency/Resident Care Director to promote the continuity of your care.
Move-out may be required if:
- You fail to meet your obligations under the Residency Agreement. This includes your, obligations to make payments in a timely manner and to abide by Cedar Mountain Commons' rules, or
- Your health status is no longer considered "chronic and stable" and you require a higher level of care than we are able to provide (such as 24 hour skilled nursing supervision, or intensive care or therapy) or
- Your behavior and/or actions pose a risk to your own health, safety or comfort or the health, safety or comfort of other residents and staff.

Copies of our Residency Agreement, Assisted Living Services Agreement, Resident Handbook and Assisted Living Residents' Bill of Rights are available upon request. These documents provide detailed information about contractual arrangements and the rights and responsibilities of the resident and Cedar Mountain Commons.
