| Cedar Mountain Commons offers up to 8 (eight) hours of personal care and nursing services per week as part of the basic monthly fee. Additional services are available at additional costs. These services are described below. |

Up to eight (8) hours of assistance per week to include, but not be limited to:
- Assistance with bathing/showering, dressing
- Medication management/reminders
- Meal-time checks
- Participation in wellness clinics, screenings, education
- Access to resident care department 24-hours per day
- Assessment of recreation and leisure needs
- Comprehensive/individualized care plan
- Regular assessments conducted by professional nursing staff to maximize function and quality of life

We Guide
(Level 1)
Cost:
$600/month |
Up to twelve (12) hours of assistance per week to include, but not be limited to:
- Assistance with bathing/showering, dressing
- Medication management/reminders
- Meal-time checks
- Participation in wellness clinics, screenings, education
- Access to resident care department 24-hours per day
- Assessment of recreation and leisure needs
- Comprehensive/individualized care plan
- Regular assessments conducted by professional nursing staff to maximize function and quality of life
- Night time safety check
- One load personal laundry per week

We Assist
(Level 2)
Cost:
$1,200/month |
Up to sixteen (16) hours of assistance per week to include, but not be limited to:
- Assistance with bathing, showering, dressing and grooming
- Medication management/reminders
- Meal-time checks
- Participation in wellness clinics, screenings, education
- Access to resident care department 24-hours/day
- Assessment of recreation and leisure needs
- Comprehensive/individualized care plan
- Regular assessments conducted by professional nursing staff to maximize function and quality of life
- Daily/nightly safety checks
- Two loads personal laundry per week
- Escort service to activities and meals

(Level 3)
Cost:
$1,800/month |
Up to twenty (20) hours of service per week to include, but not be limited to:
- Assistance with bathing, showering, dressing and grooming
- Medication management/reminders
- Meal-time checks
- Participation in wellness clinics, screenings, education
- Access to resident care department 24-hours per day
- Assessment of recreation and leisure needs
- Comprehensive/individualized care plan

- Daily/nightly safety checks
