The outreach component is an innovative program implemented to bridge the transition from traditional office-based treatment to a client’s life experience in the community.

Outreach services are integral to the recovery process and allow clients and the case manager to identify goals and ways to reintegrate into the community and to resume meaningful activity after struggling with mental health symptoms. Our expanded clinical outreach program includes a clinician; a case manager, peer counselors and a vocational specialist to greater serve the developmental needs of our young adults.

The Outreach Program within Young Adult Services aims to:
  • Increase participation by the client in educational, vocational and social activities (including monthly social events and periodic family events).
  • Provide individualized treatment and support within the community utilizing the abilities and goals of the client.
  • Provide family support, therapy and education whenever needed.
The Outreach Program is made possible through funding from the Department of Mental Health and Addiction Services.

The Outreach Program does have limited ability to provide early intervention services for young adults and families with referral directly from the community.
