Application Information

All application materials should be submitted through the PHORCAS system

Each applicant should submit:

  • A Letter of Intent
  • A Curriculum Vitae
  • Three letters of Recommendations
  • An Official Transcript from the School of Pharmacy

Requirements

  • Candidates must register through the ASHP match process
  • Candidates must submit information through PHORCAS
  • Candidates must complete a PGY I experience or received an equivalence work experience certification from ASHP
  • Be licensed in the state of Connecticut by the start of the residency
