Surgical Critical Care Fellowship - Curriculum/Overview

Simulated central line training
The primary goal of the Critical Care Fellowship Program is to train expert clinicians. The Fellowship is approved for 4 positions in Surgery and 1 in Anesthesiology. The year will be spent with direct clinical responsibility in the SICU. The fellows will manage patients under the supervision of the Attending Intensivists and will have principle responsibility over rotating house officers from the Departments of Surgery, Anesthesiology and Emergency Medicine. The fellows will be expected to provide in-depth cognitive and technical teaching to residents and occasionally medical students. Surgery Critical Care texts will be provided. 
Rotations will include:
  • 4-5 months in the General Surgical ICU
  • 2-3 months in the Cardiothoracic ICU
  • 1 month in the PICU
  • 2-3 months in the Neurosurgical IC
  • 2 weeks with Palliative Care Medine
  • 2 weeks in difficult airway management
Depending upon the needs of the individual trainee, rotations in the MICU, CCU and NICU are also available. At least one fellow will be assigned to the OR on Friday mornings. An elective month is set aside for a wide variety of rotations including operative rotations. Fellows will also undergo Critical Care Simulation training and may have the opportunity to take the ATOM course (Advance Trauma Operative Management).
 
The fellows will also become more than proficient in many ICU procedures and skills including the use of continuous CO/RVEF/RVEDV/SVO2 pulmonary artery catheters, esophageal doppler monitoring, bronchoscopy, ultrasound guided central venous access and percutaneous tracheotomies. Added exposure also exists to unique therapies such as usage of inhaled nitric oxide and multi-chamber hyperbaric oxygen.
 
The fellow will have an opportunity to participate in ongoing clinical protocols if desired. There exist a wide variety of research interests in the Department of Surgery/Critical Care. Research is critically reviewed and new ideas are discussed at the monthly research seminar. The fellow will be instructed in experimental design of clinical protocols as well as statistical analysis.
 
Fellows will also acquire additional knowledge in the areas of patient safety, palliative care and administration.
 
Call coverage is approximately six (6) calls per month with strict adherence to ACGME work hour guidelines.
 
At least four Surgical Critical Care conferences are held weekly. Conferences will include:
  1. Didactic lectures as well as case presentions
  2. Weekly CCM morbidity and mortality conference
  3. Monthly multidisciplinary trauma/critical care medicine conference
  4. Monthly journal club
  5. Monthly research conference
  6. Monthly CORE lectures


Daily multidisciplinary rounds in the ICU include: Attending Intensivist, ICU Fellow(s), Mid-Level Practioner(s), 1st and 2nd year residents, nursing staff, pharmacy, respiratory therapy, and nutritionist.
Weekly multidisciplinary rounds also occur for nutrition and additional patient care (palliative care, social services, PT, OT, case coordination).

Graduates of the program will be eligible to take the certifying examination in surgery critical care. A two-year fellowship combining surgical critical care and trauma is also available to qualified applicants.
 
Vacation
 
All fellows are given three weeks paid vacation per year. An additional week can be used for a professional meeting paid for by the department. Additional time is allowed for interviews and abstract presentations.
 
Summary
 
The Surgery Critical Care Fellowship at Hartford Hospital offers a wide range of clinical and scientific exposure to critical care as well as experience in clinical research and administrative duties for the complete management of a Surgical Intensive Care Unit. The full time Critical Care staff is dedicated to training physicians to become expert surgical intensivists providing the fellows the necessary skills to become effective critical care educators of nurses, house officers, and other physicians.
