The Helen & Harry Gray Cancer Center is one of eight recipients selected by the LIVE
STRONG Community Program in their 2008 grant cycle. The grant awarded will span three years with a goal to develop a unique Survivorship Patient Navigation Program. This exciting program provides specialized care to our cancer survivors.
This LAF-funded project facilitates a positive transition from the end of active cancer care to optimal wellness after treatment. As part of a multidisciplinary, integrated team, the Survivorship Progam provides comprehensive, coordinated after-care for breast cancer patients treated at Hartford Hospital.
The goal of the Survivorship Nurse Practitioner visit is to develop and deliver a comprehensive, effective and meaningful care plan to each survivor. During the initial visit, the patient, along with the patient's family or caregiver, will receive:
- A Breast Cancer Treatment Summary that will provide detailed information about each patient’s specific breast cancer treatments
- A Breast Cancer Survivorship Care Plan, outlining information on healthy living, signs and symptoms of recurrence, and potential late effects of treatment
- Assistance with coordination of care between primary care physicians, medical/radiation oncologists, and other specialists
- Information about available support programs, wellness programs, and referrals to other health care professionals and services as necessary
- Education and assistance with setting goals for healthy living
In addition, by encouraging active patient and caregiver participation in the plan of care, post-treatment anxiety may be reduced and quality of life enhanced for Hartford Hospital's breast cancer survivors.
Online Patient Portal
Once you have received your treatment summary and care plan, we invite you to access your records through our secure online patient portal. A username and temporary password are included in your survivorship packet to help you electronically access your individualized treatment summary, follow-up care schedule and educational materials. You can also complete any questionnaires assigned to you on this site.
We encourage you to schedule your survivorship appointment online using the Schedule An Appointment button or by calling 860-545-4681.

The ACS Patient Navigator helps identify the barriers patients may have in accessing treatment and works in partnership with the cancer clinical team to connect patients to programs and services available at the hospital, through the ACS and in the community. Some of the ways that we can help…
