We must change the way we deliver care, investing heavily in information technology and forming stronger partnerships between institutions and physicians in order to provide the coordinated, quality care that will enable us to manage chronic disease, promote community wellness and improve population health.

The 2014 Environmental Assessment provides a deep dive into six key areas that are impacting our work every day and that we must consider as we plan for the next three-to-five years.
